A birth plan is not really a plan. It is a short summary of what matters to you, written down while you are calm, so that other people can read it quickly when you are not.

The problem with most templates is length. They run five pages, cover every conceivable scenario, and never get read by the person who needs to read it. This one is built the other way around: detailed worksheets for you to think it through, and a single clean page to actually hand over.

How to use it

Fill in the worksheets over a few sittings, then condense the answers onto the one-page summary. Bring that page to an appointment and talk through it with your provider before you finalize it. Print four or five copies — one for your bag, one for your partner, and spares for whoever is in the room.

A note on scope

This is an organizational template, not medical guidance. It does not tell you what to choose or what is safest. Every preference here is a starting point for a conversation with the provider who knows your history.

Questions

What should a birth plan actually fit on?

One page. Nurses and midwives are reading it during a shift change, and a five-page document does not get read. This template gives you detailed worksheets to think it through, then a single summary page to hand over.

When should I fill it out?

Most people work on it somewhere around 32 to 36 weeks, then bring it to an appointment to talk through. Earlier is fine — you will likely revise it.

Does it cover an unplanned cesarean?

Yes. There is a section for preferences if plans change, which is the part people most often wish they had written down.

Is this medical advice?

No. It is an organizational worksheet. Every preference on it is something to discuss with your own provider, who knows your history and your situation.
